Proof Perks Membership Terms
Proof Perks is a recurring monthly membership. By enrolling, you authorize Proof Medical Spa LLC to charge $199 to your payment method on file each month on your enrollment date. The membership continues automatically until you cancel.
Proof Perks requires an initial commitment of three monthly payments. After your third payment you may cancel at any time by emailing hello@proofmedspa.com. Cancellation takes effect at the end of your current billing month.
Banked credit never expires and is not forfeited if you cancel. Credit is not redeemable for cash and is not transferable. Payments already made are non-refundable. Member discounts apply only while your membership is active and may not be combined with other promotions, packages, or discounts.
Proof may change membership pricing or benefits with at least 30 days’ written notice. You may cancel before a change takes effect, subject to the initial three-month commitment.
