Membership Agreements
Proof Perks has three programs. Each is a separate enrollment with its own terms.
The Proof Pass
$199, one time. Twelve months of member pricing. No monthly charge.
By purchasing, I agree to the following terms with Proof Medical Spa LLC (“Proof”).
One-time purchase. I authorize Proof to charge a single payment of $199 to my payment method. This is not a recurring charge, and Proof will not bill me again for this program.
Term. My Pass is active for twelve months from the date of purchase. It does not renew automatically and will not convert into a monthly membership. Member pricing ends on the expiration date unless I purchase a new Pass or enroll in another program.
Member benefits. While my Pass is active, I receive:
- $1 off per unit on Botox and Dysport, from the first unit
- 10% off retail products
- 5% off treatment packages
Member pricing applies only while my Pass is active and may not be combined with other promotions, packages, or discounts. Where more than one discount could apply, the largest single discount applies. Discounts are not added together.
No banked credit. The $199 purchase price buys member pricing. It is not credit toward services or products and cannot be applied to a treatment or a retail purchase.
Non-refundable. The purchase price is non-refundable.
Non-transferable. My Pass is tied to my account at Proof and may not be transferred, shared, or sold.
Changes. Proof may change the benefits offered with Passes sold after the date of any change. The benefits attached to my Pass will not be reduced during my twelve-month term.
Acknowledgment. I have read and agree to these terms.
Fool Proof
$199 per month. One treatment every month.
By enrolling, I agree to the following terms with Proof Medical Spa LLC (“Proof”).
Monthly membership. I authorize Proof to charge $199 per month to my payment method on file, beginning on my enrollment date and recurring on the same day each month until I cancel.
Monthly treatment. Each month I may schedule one of the following, each including a complimentary LED add-on:
- Deluxe HydraFacial® (60 minutes)
- Proof Signature Facial (60 minutes)
- Chemical peel
The monthly treatment does not roll over. It must be used within the billing month in which it is earned. Unused treatments expire at the end of that month, are not banked as credit, and are not refunded. Scheduling my appointment within the month is my responsibility.
Member benefits. While my membership is active, I also receive 10% off retail products and 10% off treatment packages. Member pricing applies only while my membership is active and may not be combined with other promotions, packages, or discounts. Where more than one discount could apply, the largest single discount applies. Discounts are not added together.
Initial commitment. My membership requires an initial commitment of six monthly payments. After my sixth payment I may cancel at any time by notifying Proof in writing at hello@proofmedspa.com. Cancellation takes effect at the end of my current billing month. Payments already made are non-refundable.
Payment method. I am responsible for keeping current payment information on file. If a payment fails, Proof may suspend member benefits until the balance is resolved. A month in which payment is not completed does not earn a treatment.
Scheduling. Treatment availability is subject to the schedule. Proof will make reasonable efforts to accommodate my preferred time but does not guarantee a specific day, time, or provider.
Changes. Proof may modify membership pricing or benefits with at least 30 days’ written notice. I may cancel before a change takes effect, subject to the initial six-month commitment.
Acknowledgment. I have read and agree to these terms.
Bullet Proof
$249 per month, banked as credit. Member pricing across the menu.
By enrolling, I agree to the following terms with Proof Medical Spa LLC (“Proof”).
Monthly membership. I authorize Proof to charge $249 per month to my payment method on file, beginning on my enrollment date and recurring on the same day each month until I cancel.
Banked credit. Each monthly payment is credited to my member account. Credit may be applied toward any service or retail product offered by Proof. Unused credit accumulates month over month, does not expire, and is not forfeited if I cancel. Credit is not redeemable for cash and is non-transferable. Member pricing determines the price of a service or product, and my banked credit is then applied as payment.
Member benefits. While my membership is active, I receive:
- $100 off each filler or Sculptra treatment, with no annual limit
- $1 off per unit on Botox and Dysport, from the first unit
- 15% off retail products
- 15% off treatment packages
- One complimentary vitamin B12 injection each month
Member pricing applies only while my membership is active and may not be combined with other promotions, packages, or discounts. Where more than one discount could apply, the largest single discount applies. Discounts are not added together.
The monthly B12 injection does not roll over. Unused injections expire at the end of the billing month and are not banked as credit.
